Implants unfortunately don’t last forever. At some point, they will need attention due to implant rupture, age, or suspicion of leaking. Many women are concerned that their breasts will look unattractive and deflated if they have their implants removed without getting new implants—yet at the same time, they don’t really want implants anymore. Cassileth Plastic Surgery uses an innovative “internal lift” technique to put the hanging breast tissue back where it belongs. Depending on the amount of loose skin that you have, you may also need a lift, known as a mastopexy, to enhance the appearance of breasts after breast implant removal.
Most plastic surgeons perform a removal and lift using a full lollipop incision. Dr. Cassileth and Dr. Killeen, however, perform implant removal surgery through a tiny incision hidden in the inframammary crease (the fold below the breast). The internal lift is performed through the a small incision hidden below the breast
The skin of the lower breast is first separated from the breast tissue, allowing the breast to be lifted without cutting the skin. Depending on the degree of the lift, tissue is taken from the lower half of the breast and tucked under the central breast to create an “autoaugmentation” of the breast. This creates volume for the upper half of the breast.
Then the lower half of the breast is brought together, creating a neat breast that is pulled up above the level of the natural crease under the breast for a perky, lifted breast. This is just one of the methods we use to leave our breast implant removal patients with tiny incisions and beautiful, natural results.
If you have symptoms of breast implant illness (BII), or if you have capsular contracture, you may want to have your breast implant capsule removed in a procedure called a “capsulectomy.” This is performed during breast implant removal to address any implant-related complications and restore breasts to a more appealing state while minimizing health risks. For patients who have biofilms or free silicone, it is important to remove the entire capsule. The capsule is sent for pathology, microbiology, and for PCR.
Many of our patients with breast implant illness autoimmune problems find that that their symptoms improve as early as one week after surgery.
Fat grafting can be performed after breast implant removal for patients who want to restore their natural breast to a fuller shape. This grafting can be performed during the breast implant removal surgery and can double the native breast size. For excessive skin, a mastopexy can be performed that can raise the nipple position and remove the loose skin.
