Can You Have a Breast Reduction While on Accutane?

The current evidence does not support delaying a breast reduction if you are on Accutane. And that is my personal practice.

Accutane and Breast Reduction: Do You Need to Wait?

For many years, the standard recommendation was to stop Accutane (isotretinoin) at least six months before surgery. The concern was that the medication would cause significant wound healing problems. Patients were told to either finish their entire course first or pause it well in advance.

What the Current Evidence Shows

We now have good evidence that Accutane does not impede wound healing. The old six-month rule is no longer supported by the data.

This means it is completely reasonable to proceed with breast reduction surgery while on Accutane. There is no need to delay.

My Personal Practice

In my practice, I typically do not delay surgery for patients on Accutane. The current evidence supports this approach.

That said, you will find surgeons who still prefer to wait — and that's their prerogative. If your surgeon recommends delaying, it's worth having a conversation about the reasoning and the latest research.

  • The old belief that Accutane impairs wound healing is not supported by current evidence
  • You do not need to delay breast reduction if you're on Accutane
  • Some surgeons still follow the older guidelines — discuss it openly with your provider
  • This applies to breast reduction specifically, but the wound healing data is broadly applicable
