What is a Breast Lift?

Breast lift surgery raises and reshapes sagging breasts by removing excess skin and rearranging the breast tissue into a higher, perkier shape. Unlike augmentation or reduction, a breast lift primarily addresses the position and contour of your existing breast tissue, repositioning nipples and areolas for a youthful, rejuvenated appearance.

Every lift is a trade: a better, longer-lasting shape in exchange for a scar on the breast. Sagging is linked to age, weight loss, pregnancies, smoking, and breast size (not, as it turns out, to wearing a bra).

Most of Dr. Killeen's breast lifts are done without implants. A lift alone creates a natural slope up top and a rounded bottom; an implant can be added for more volume or upper fullness, but an implant by itself does not lift a sagging breast. You may benefit from breast lift surgery if you:

Physical Symptoms

  • Experience sagging or drooping breasts (ptosis)
  • Notice nipples pointing downward or sitting below the breast fold

Sagging is graded by where the nipple sits relative to the crease under the breast. With grade 2 or grade 3 ptosis, a lift is almost always needed. With mild (grade 1) ptosis, it depends on your goals, and added volume can sometimes stand in for a lift. Customized Techniques for Your Ideal Results

The lift pattern depends on your anatomy and how much lift you need. As Dr. Killeen puts it, the lift chooses the patient:

Vertical ("Lollipop") Lift

An incision around the areola and straight down to the crease. It provides a significant lift and reshaping, and it is the pattern Dr. Killeen uses most often.

Inverted-T ("Anchor") Lift

Adds an incision along the crease under the breast. It is used for very low or very large breasts and after major weight loss, when more skin needs to be removed.

Minimal-Scar Lifts

Periareolar ("donut") and crescent lifts remove skin only around the areola. Over time the areola tends to stretch into an oval and the lift does not hold, so Dr. Killeen does not use them for a breast lift. Nipple Position

The new nipple position is planned from measurements and your proportions, and you review the markings before surgery. A nipple placed too high is far harder to correct than one placed slightly low, so the plan errs on the side of low. See how nipple position is planned.

Internal Support

For patients whose tissue is already weak, such as younger women who are droopier than expected, or when a previous lift has drooped again, Dr. Killeen may place a dissolvable surgical mesh low in the breast to support the crease. It dissolves over one to two years, leaving a layer of collagen. All of these scaffold products are used off-label in the breast. More on the internal bra. Breast Lift Surgical Techniques

Performed at our accredited Beverly Hills surgery center or Cedars-Sinai, your comfort and safety are paramount. Surgery typically takes 2-4 hours under general anesthesia with meticulous, precise techniques, and it is usually an outpatient procedure, so you go home the same day.

Your Recovery Timeline

Pain after a breast lift is usually on the lower end for plastic surgery. A typical timeline:

Breast Lift Recovery Process
  1. First Week

    A long-acting numbing medication covers the first few days, and most patients get by with ibuprofen after that. Drains are typically not needed, and many patients go home with a Prevena incision dressing that stays on for about a week.
  2. Weeks 1-4

    Most patients with desk jobs return to work after about a week; physical jobs can take up to a month. No lifting over 5 to 10 pounds and no exercise for about four weeks, and consistent bra wear helps the lift settle.
  3. Weeks 4-6

    Scar care with silicone gel or sheets and sun protection begins once the initial healing is done. Early on, the breasts can look square or flat on the bottom; this square phase is temporary.
  4. 6-12 Months

    The shape settles over six months to a year. Scars often look good early, turn pink and thicker for a while, then fade over six months to two years.

Risks of Breast Lift Surgery

A breast lift is safe because it is elective, done in a controlled setting, and done on healthy patients; risk rises when it is combined with other operations, especially a tummy tuck. Learn how safe a breast lift is.

Possible Risks

  • Delayed healing where incisions meet, especially at the T-junction, and stitches working their way out
  • Nipple and areola blood-supply problems, which often look worse than they are and are usually superficial
  • Asymmetry: each side can heal differently, and the goal is meaningfully better, not flawless
  • Shape changes, such as a torpedo-shaped breast if the central tissue stretches after surgery
  • Fat necrosis, which can feel like a firm lump
  • Changes in nipple sensation
  • Effects on breastfeeding
  • Scars, which fade over time but are permanent
  • Sagging again over time, since a lift is not permanent

When to Call

Normal swelling and soreness improve day by day. Swelling, pain, or redness that gets worse should be checked right away.
