Can You Fix a Diastasis Without a Tummy Tuck?

The answer is absolutely yes — but you may want to consider doing the tummy tuck anyway. Here's why.

What Is a Diastasis?

A diastasis recti occurs when the rectus abdominis muscles — the "six-pack" muscles in the center of your abdomen — get pulled apart. This causes the abdomen to look very round, and if you lean back, you may notice a pointed or ridged shape down the middle.

Muscle Repair Without Skin Removal

You can absolutely have the muscles repaired without removing any skin. The repair can be done through incisions with direct vision or with the help of an endoscope. The surgeon brings the separated muscles back together in the midline.

However, the aesthetic results are often not ideal unless the surgeon does significant tissue releasing. Here's the problem: if the muscles were spread far apart and you bring them back together, the skin over the top becomes a bubble that creates a strange contour. To avoid this, the skin needs to be separated all the way out to the sides over the oblique muscles so it can lie flat and smooth.

The Loose Skin Problem

There's another issue that's easy to underestimate. When you have a diastasis, the skin looks relatively tight because it's stretched over a round, protruding abdomen. Once the muscles are repaired and everything is brought back in, that same skin now looks quite loose.

Unless you've seen diastasis repairs done without skin removal, it's hard to appreciate just how much excess skin can result. Most patients with a significant diastasis really benefit from skin removal — meaning a full tummy tuck — in addition to the muscle repair.

The Bottom Line

Yes, you can have your muscles repaired without having skin removed. But make sure you have a thorough conversation with your plastic surgeon about what to expect after surgery. In many cases, addressing both the muscles and the skin gives a much better overall result.
