What Is a Prevena Wound Vac? A Complete Guide to How It Works and Who Needs One

Prevena vacs are one of my favorite adjunctive tools for helping my patients heal. I've been using them for about eight years in my practice and I think they're amazing. Let me show you what one actually is, how it works, who benefits most, and the practical stuff, how to get one and what it costs.

Wound vacs have been around in medicine for a very long time, traditionally to heal open wounds.

The classic scenario: you're in a car accident and a big chunk of tissue is missing from your thigh. We can put a sponge in the wound, cover it with plastic wrap, and attach it to a machine that gently suctions. That negative pressure helps the wound heal faster.

The catch with those traditional vacs: you couldn't use them over normal, intact skin, because they would actually break down and harm the skin. That limitation is exactly the problem Prevena solves.

Prevena is a wound vac that's safe to use on intact skin. You get the benefits of that negative pressure, but applied over your closed surgical incisions rather than an open wound.

So instead of putting a traditional dressing on after surgery, we put on a vac. Here's what it consists of:

• A customizable sponge we can cut to any size or shape we need
• A layer of plastic wrap (the same material used for hospital wound vacs)
• A small plastic "lily pad" with a tube, connected to a small pump
• The pump is single use, unlike hospital vacs, it isn't cleaned and reused between patients

It stays on for seven to nine days after surgery.

During that week the vac is on, the gentle negative pressure dilates (enlarges) the blood vessels under your skin. That does two valuable things:

1. Brings extra blood flow to the area, so the incision heals better.
2. Lets those vessels absorb fluid from the tissue, so you get rid of swelling faster.

More blood flow and less swelling is a genuinely powerful combination for an incision that's under tension or at risk.

Not every patient needs a Prevena for every type of surgery. We generally reserve them for two groups.

In my practice, I use Prevena for:

• Breast lifts
• Breast reductions
• Breast reconstructions
• Tummy tucks

These procedures traditionally carry an elevated risk of wound healing problems, often right where incisions meet, like the T-junction after a lift. I find my patients' risk is very low, and I think the Prevena is a big part of that.

Patients who heal poorly do great with a Prevena even for surgeries that aren't typically high-risk. That includes:

• Diabetes
• Hypertension
• Immunosuppressed patients (transplant patients, autoimmune disease)
• Patients with prior radiation

All of those can cause poor healing, so the extra support matters.

You can also have one simply because you want one, since it does help healing regardless.

Here's a bonus that surprises people: Prevena also seems to make scars look better. Because of that, it's used in the treatment of keloid scars. When I'm revising patients who have keloids or terrible hypertrophic scars, I often use a Prevena to relieve tension and improve the final result. If you have a history of bad scarring, this is well worth asking your surgeon about.

Start by discussing it with your doctor, they'll let you know what's available. In practice:

• If you're having an insurance-covered surgery, the hospital may already carry Prevena. Both Cedars-Sinai, where I operate, and my QUAD A accredited surgery center stock them.
• If your hospital or surgery center doesn't carry it, you or your surgeon can purchase one and bring it in. Because it isn't used during surgery, it's just a dressing, most hospitals will let you fill out paperwork to use your own.

The honest answer on whether you'll pay out of pocket: maybe.

• Insurance-covered procedure + the facility carries it → they bill insurance, and my patient doesn't pay out of pocket.
• Cosmetic procedure, or the facility doesn't carry it → you may pay out of pocket.

Cost varies by size and model, roughly $750 for the cheapest up to close to $2,000 for the largest.

A Prevena vac is a negative-pressure dressing that's safe on intact skin, worn over your closed incisions for seven to nine days. The gentle suction dilates blood vessels, which increases blood flow (better healing) and pulls out fluid (less swelling).

It's most valuable for higher-risk procedures (breast lifts, reductions, reconstructions, tummy tucks) and for patients who heal poorly (diabetes, hypertension, immunosuppression, prior radiation), and it appears to improve scars, so it's useful in keloid revision too. You can often get it billed to insurance, or buy one (roughly $750 to $2,000) and bring it in.
